Baby Girl Young was born today at 37 weeks gestation via c-section. Mrs. Young is a 35 year-old G2 now P2 with a prenatal history significant for previous c-section (for failure to progress) and chronic hypertension with superimposed pre-eclampsia. During pregnancy Mrs. Young took Labetalol as well as prenatal vitamins. She initiated prenatal care in the first trimester of this pregnancy and has had consistent prenatal care with an obstetrician during her pregnancy. Mrs. Young is married and she and her husband reside together with their 5 year old son. Mrs. Young presented to her obstetrician?s office with a head ache and it was noted that she had significant blood pressure elevation. She was admitted to L&D where her BP remained elevated. She was started on magnesium sulfate and the decision was made to perform a c-section. Mrs. Young?s labs are as follows: Blood type A+, RPR NR, HepB neg, HIV NR, Rubella immune, GBS negative. At birth Baby Girl Young had delayed cord clamping for 60 seconds. ROM occurred at delivery and fluid was clear. The baby only required drying and routine care after birth and had Apgars of 8 and 9. She weighed 2000 g, length 46 cm, head circumference 33.5 cm. She was placed on her mother?s chest for skin to skin contact in the OR. The transition nurse calls the NICU when Baby Girl Young is one hour of age and reports that her axillary temperature is 96.5 and her bedside glucose is 17. She is transferred to the NICU for further evaluation.You are the NNP responsible for Baby Girl Young upon admission. You perform a physical exam and note that the baby is active with tone appropriate for gestational age of 37 weeks. You find her to be jittery. She has no increased work of breathing and you hear no murmur. Her abdomen is flat and soft and she does not have hepatosplenomegaly. She is adequately perfused with equal femoral and brachial pulses. You note no dysmorphic or unusual features. You note the admission vital signs in the EMR: Temperature axillary 96.8, heart rate 154, respiratory rate 52, blood pressure 60/40 (mean 47). Oxygen Saturation 97% in room air (probe on right hand).
